From living wills to POST: Everything you always wanted to know about Advance Care Planning documents

From advance medical directives, to DNRs and POST - join Honoring Choices in the fifth of six bimonthly learning sessions for 2020. We will be learning from VCU Health’s expert ACP Coordinator, Candace Blades, JD, RN, as she helps us take a deeper-dive into the nuances of the class of advance care planning documents available to patients today.

Goals

  • Review the history of the advance medical directive (AMD) and how this influences patient perceptions and completion
  • Review capacity and incapacity
  • Review the Virginia AMD form and learn how to fully and accurately complete it
  • Understand the difference between the Virginia AMD and other ACP documents, such as a do-not-resuscitate (DNR) order, the durable-do-not-resuscitate (DDNR) order, and the Virginia Physician Orders for Scope of Treatment (POST)

More on our presenter: Candace Blades, JD, RN, is a graduate of the University of Virginia School of Nursing and the University of Maryland School of Law. She practiced law in a variety of health care settings before returning to health care practice 10 years ago. Candace is an experienced critical care practitioner and is passionate about supporting patients, their families, and the care team through the difficult but sacred end-of-life journey. Candace is the Advance Care Planning Coordinator for VCU Health System and a Respecting Choices® First Steps® and Advanced Steps® certified facilitator and instructor.

Primary audience: All trained Respecting Choices facilitators and ACP ambassadors, interested HCA VA and VCUHS employees and students, and RAM members

Cost: Free to HCA VA, VCUHS, and HCV Community trained First Steps facilitators, HCV trained community ambassadors, HCA VA and VCUHS employees and students, and RAM members. Please contact HCV staff or your health system ACP Coordinator for your free registration code. All others, $10. See separate event registration option to pay for the entire series at a discounted price.

Advance registration is required. Coffee will be provided.
