What if a sudden illness or injury left you unable to speak for yourself? Who would you want to speak for you? What would you want them to know about your values and wishes?

Advance Care Planning (ACP) is a conversation process about the medical care you would want if you could not speak for yourself, and needed others to make decisions for you. Advance care planning helps you think about health care choices you may have to make in the future; talk about your choices with your doctor and your loved ones; and make a written plan for the future (advance directive).

Goals:

  • Understand advance care planning and the critical steps of this process
  • Recognize common pitfalls and errors that occur during advance care planning, and how to avoid them
  • Learn about the resources available to assist individuals in developing a thoughtful, person-centered advance care plan
